Insurance & Payment
Note: Please call your insurance company to verify your benefits, co-pay/co-insurance, and deductible prior to scheduling your appointment, as I am unable to guarantee your coverage. Although I am in network with several major insurance carriers, you may have a specific policy that does not cover my services. If you do not check with your insurance company prior to your first appointment, you will be charged for any balance that is not covered.
I am in network with Blue Cross Blue Shield (Regence, Anthem), Moda, PacificSource, and Aetna. Please note that I am unable to see patients who have Medicare and/or Medicaid (Oregon Health Plan, Molina Health, etc.).
If I am out of network with your plan, your insurance company may still be willing to pay for my services. In this case, you will be charged for your visit at the time of service and I will provide you with a “Super Bill” (itemized receipt) that you can submit to your insurance company for reimbursement. Each plan varies with regard to their coverage of out-of-network services, so please call the number on your insurance card to inquire.
