Note: Please call your insurance company to verify your benefits, co-pay/co-insurance, and deductible prior to scheduling your first appointment, as I am unable to confirm that your specific policy will cover my services. Even if you have a policy with one my contracted insurance carriers (listed below), your specific plan may have a limited provider network. If you do not check with your insurance company prior to your first appointment, you will be charged for any balance that is not covered by your plan. I am currently in network with Blue Cross Blue Shield (Regence, Anthem), Moda, PacificSource, and Aetna. Please note that I am unable to see patients who have Medicare and/or Medicaid (OHP, Molina, etc.), even if the plan is a form of secondary coverage.
If I am out of network with your plan, your insurance company may still be willing to pay for my services. In this case, you will be charged for your visit at the time of service and I will provide you with a “Super Bill” (itemized receipt) that you can submit to your insurance company for reimbursement. Each plan varies with regard to their coverage of out-of-network services, so please call the number on your insurance card to inquire.
