Booking an Appointment
Please read this page thoroughly for booking instructions and important details about insurance/payment.
PROSPECTIVE PATIENTS: Please use the booking link below to schedule a free 10-minute phone consultation so that we can discuss your treatment needs/goals prior to scheduling a new-patient evaluation. You will receive an email confirming your appointment once I’ve reviewed your request, and I will reach out to you at the scheduled date/time.
NEW PATIENTS: When you are ready to schedule your first appointment, please click the scheduling link below and select the Initial Evaluation option at your preferred time. After submitting your request, you will receive an email with instructions to complete an intake packet which includes a health history questionnaire and treatment consent forms. Please complete this promptly as your requested appointment date/time will not be confirmed until the packet is submitted, and you may lose your desired date/time. The packet must be completed at least 48 hours prior to your first appointment, otherwise your appointment will be canceled/rescheduled and you will be charged a late cancelation fee of $250.
EXISTING PATIENTS: You may use either the appointment scheduling or patient portal link below to schedule a follow up appointment. If you have not yet set up your portal account, you can do so by clicking the portal link below and selecting “Don’t have an account”. Appointment requests for existing patients will be confirmed automatically.
***Important Information***
Payment Method: A valid credit card (not an HSA/FSA card) must be stored in your account to book appointments at Chrysalis Mental Health. Your intake packet will provide directions for making an HSA/FSA card your preferred payment method; this can be done before or during your first appointment or at any point thereafter. Any card information that you provide will be stored in an encrypted and HIPAA-compliant platform; your card details are not accessible to me, my billing company, or anyone else.
Insurance Benefits: If you are a new patient, please call the number on your insurance ID card and verify that your plan will cover my services as I am unable to call on your behalf or guarantee your coverage. While I am in network with several major insurance carriers, you may have a specific policy that limits where you can seek care. For example, if you have an “EPO” policy with one of the insurance carriers with which I’m in network, it is unlikely that my services will be covered. If you do not verify your coverage with your insurance company and my services are not covered by your policy, you will be charged full service rates. This also applies to patients whose insurance policy changes after they establish care with me. When you call your insurance company, please reference my National Provider Identification (NPI) number, 1346858115 and my Employer ID Number (EIN), 991983559.
Visit Type and Format: Evaluations for new and existing patients will be automatically scheduled via telehealth. If we have agreed to an in-person appointment via email, portal message, or during an appointment, I will manually change your visit format after you have scheduled.
